  5. Post-hospital discharge: Organizing of medication, equipment, service providers, to avoid re-admission. 

Health Promotion and Education
  1. Patient Education: Educating patients and their families about disease management, medication usage, lifestyle changes, and preventive health measures.
  2. Health Screenings: Conducting screenings for conditions such as hypertension, diabetes, and cholesterol to identify health issues early.
  3. Wellness Programs: Developing and implementing wellness programs to promote healthy living and disease prevention, including nutritional counselling.​
  4. Screening and Monitoring: Regularly monitoring patients for potential health issues and providing early interventions.
