What happens when I'm discharged from hospital? It's a difficult question for patients to grapple with, especially when facing a wait list for Long Term Care or ALC beds. It means that more seniors and their families will be given limited and often unsatisfactory options, especially those with more acute care needs. With the high demand for hospital beds, the Discharge Planners face a lot of pressure to move patients out, and retirement residences are generally not equipped to handle returning residents with significant care requirements. What support do you have in place, post hospitalization? It's often a rush to get the appropriate equipment, medications and caregivers in place in a timely way. It's the hospital Discharge Planner's biggest challenge, which transfers to a retirement residence or household that is ill-prepared to meet the increased needs. A Nurse Care Manager will proactively make arrangements with or for you, putting many pieces of the puzzle together and doing what is required to have you on the right path.

REsolving issues when publicly-funded community support is lacking

In Canada, seniors and many others are entitled to community support services, but it doesn't always happen as it should, leaving some patients with no care or less than required. Often the result is readmission to hospital due to an error or accident that occurred due to lack of support and oversight. Nurse On Board advocates on your behalf to facilitate the community care you are entitled to, and to ensure the quality of care is acceptable.

the retirement residence offers some care, but not at the level the patient requires

Even if you live in a retirement residence, this doesn't guarantee you'll get the appropriate level of care. They may not be set up to provide acute care or to respond to your changing needs following hospitalization. The fact is that residences are staffed with practical nurses and they are trained to care for stable patients, whose needs are not complex. This makes it difficult for Discharge Planners to achieve their goals of getting people home. With the involvement of a Nurse Care Manager, your recovery and all that is needed to make it a positive outcome, becomes the ONLY priority at that critical juncture.

the complexity of care at hospital discharge threatens continuity and desired outcomes

Appointments. Medications. Follow-up treatments and procedures. New specialists. New services. It often gets complicated following a hospitalization. It’s exceptionally difficult for patients and families to know what is a priority, what is reasonable or unreasonable, of sufficient concern to act upon, and more. The Discharge Planner offers educational material and options for private care but is of minimal support with medical issues. Retirement residences aren't typically set up to offer specialized care, and they have minimal input and offer minimal direction regarding publicly funded community care. A Nurse Care Manager will take care of it all.

family members are overwhelmed and trying to understand their loved ones needs

Too often family members feel overwhelmed looking after a loved one’s needs during or after a hospitalization. As non-medical caregivers, they rely on advice from Discharge Planners and retirement residence staff. They may discover that, sooner than expected, their medical and care needs are not being met.  A Nurse Care Manager becomes the eyes and ears for the family, identifies unmet needs as well as gaps in care, offering reliable advice, guidance and direction.

ALC rates and hospital costs are overwhelming

Is access to Long Term Care (LTC) or Alternate Level of Care (ALC) an option? Are you benefitting from all of the no-fee services that you can? Retirement residences staff don't have the time or interest in making that happen as they can offer 'private care' as an option, though often not an affordable solution. Your Nurse Care Manager will investigate every penny of no-fee service you're entitled to and will work hard to get you the support you need and can afford.
