By investing the time to understand your circumstances, we're able to develop a customized Plan of Care based upon your preferences and unique needs. Some are identified during your initial complementary 30-minute phone consultation, and later during the Intake Visit. After that it's your Nurse Care Manager's role to serve as your guide to having the care you need, and to stay current on any changes to your ongoing needs and goals of care.
