Community Palliative Care


Your Loved One Deserves the Best Palliative Care Possible.


Our highly experienced RNs and RPNs deliver exceptional palliative care throughout the Ottawa area. We liaise with healthcare professionals to support the best possible experience for end-of-life care.
 
We collaborate with Home and Community Care Support Services (HCCSS) and the local palliative teams to fill significant gaps in nursing services that many clients and their families face. During this difficult time, the burden shouldn’t have to fall on loved ones.
 
With third party insurance coverage, our fees are largely subsidized. Contact us to learn more.


What Our Nurses Offer:​
​
  • Shift Nursing - we can provide care in 4, 8 and 12 hour shifts, Day or Night;
  • Support of loved ones during the grieving process;  
  • Anticipation of future care needs;
  • Collaboration with healthcare providers;
  • Communication with family members/caregivers;
  • Oversight and coordination of services.​

​Principles of Palliative Care (per the World Health Organization)​

  • provides relief from pain and other distressing symptoms;
  • affirms life and regards dying as a normal process;
  • intends neither to hasten or postpone death;
  • integrates the psychological and spiritual aspects of patient care;
  • offers a support system to help patients live as actively as possible until death;
  • offers a support system to help the family cope during the patient’s illness and in their own bereavement.

​Research Indicates that:

8/10 patients would prefer to die at home, however 6/10 patients die in hospital

Nurse On Board is dedicated to meeting the wishes of our patients and their family members, whenever possible. Often this requires specialized nursing support beyond what is currently available through Home and Community Care Support Services (HCCSS). 

​With only a small number of hospice beds in Ottawa, our professional guidance and support may be exactly what is needed for those who prefer to remain at home for palliative care.
